Reporting a Work-Related Injury

If you are injured at work, the first step is to report the incident to your supervisor and the Office of Safety and Security at 407-708-2178. The incident must be reported whether you require medical care or not. After you have reported the incident to your supervisor and Security, you must fill out an Accident / Incident Report. 

The responding Security Officer will provide you with the Accident/Incident Report and is available to assist you in the proper completion of the report. The Security Officer will take the report and use it to complete the formal computerized incident report.

Once you have completed the Accident/Incident Report please contact the Office of Risk Management to complete a First Notice of Injury whether the incident requires medical care or not. 

If you seek medical care, take the First Notice of Injury to your appointment, and give it to the receptionist. Send all documents received at the medical appointment(s) to the Office of Risk Management and the insurance adjustor assigned to your case.
