DSS Student Responsibility Form - What I Need to Know

Disability Support Services (DSS) Responsibilities

You are responsible for submitting or updating your documentation as documents are not shared between departments. You are also responsible for verifying that DSS has received it and it meets the guidelines for documentation.

You must request your accommodation letters each semester. 1) Request your letters online at portal.seminolestate.edu or in person at any DSS office once you are registered for classes. 2) Stop by the office to pick up your letters for on campus classes. 3) Letters will be emailed for online classes. 4) Get new accommodation letters if your class schedule changes.

Extended time for exams/quizzes. You will usually take your exams in make-up testing. 1) Remind your professor one week before the exam to send the exam to assessment and testing. 2) Make an appointment by visiting the Assessment and Testing Center. 3) You are expected to take your exam the same day and time as the rest of the class unless you have prior approval from your professor.

If you are approved for adaptive equipment, check out the approved equipment at the beginning of each semester. Return equipment at the end of each semester to avoid holds and fines. All fines are non-refundable.

If you are approved for alternate format books: purchase the textbooks you want to request and bring them to DSS as soon as possible to ensure timely processing of your request. Processing time could take up to 3 weeks.

If you use a wheelchair for mobility, please contact the DSS office as soon as possible if you need an adjustable table or other modification.

If you are approved for a note taker, check for notes daily or weekly in the DSS office. If done weekly, you may want to arrange a dedicated day of pickup with the DSS office to ensure that your notes stay current with the class.

If you have problems with accommodations during the semester, contact DSS immediately at 407-708-2110.

Note: Your approved academic accommodations are valid for Seminole State College classes andexams only. If your degree requires an outside the college certification exam or license exam such asGED, NCLEX, etc., you will need to follow their specific procedures to request accommodations for that particular exam with the governing agency.

Academic Responsibilities

Each semester you will need to meet with an academic advisor to determine which courses to take based upon your educational plan. You should do this before your registration appointment date.

Register for classes each semester as soon as you are allowed to register. Your registration date can be found in your Student Center Portal under Registration Appointments.

Visit the Academic Success Center (located at each campus) for free tutoring. You may contact the ASC for hours of operation.

Visit the Career Development Center (located at each campus) for career exploration.

Financial Aid/Fee Payment Responsibilities

Renew your Free Application for Federal Student Aid (FAFSA) each year as early as January. Step by step instructions can be found on the FAFSA website

Accept and defer your Financial Aid or pay your tuition by the due date each semester to avoid being dropped from your classes.

Be informed regarding rules and regulations concerning Financial Aid Standards of Academic Progress (SAP). More information can be found on the Financial Aid webpage, "Standards of Academic Progress for Financial Aid".

Maintain the Grade Point Average (GPA) and completion rate that are needed for Financial Aid and academic progress.

If receiving financial aid, notify Financial Aid of any program changes and before withdrawing from courses to determine how these changes will affect your award.

Check your MySeminoleState “To Do List” frequently and contact Financial Aid if you have any questions or concerns regarding your award.

Important Information - It is your responsibility to:

Check your MySeminoleState account regularly for Holds, To Do List, and Registration Appointments.

Check your Seminole State Live Mail email account regularly for important information.

Review the college academic calendar for all college dates including first day of classes, add/drop, withdrawal deadline, last day of classes, etc. These dates are available in the college catalog.

Comply with all college academic policies and procedures in the Seminole State College of Florida catalog found on the college's website.

Comply with the Student Code of Conduct that is applicable to all Seminole State College students including students with disabilities. You may view this on the Student Affairs webpage under "Student Code of Conduct".

Graduation - It is your responsibility to:

During your final semester, apply for graduation by the deadline listed in the academic calendar.

Check with your transfer college or university regarding their student disability services’ policies and procedures several months before you plan to transfer.
