Order a Replacement Diploma/Certificate

The Request for Replacement Diploma/Certificate form should be used only to request an additional or replacement diploma/certificate.

The Request for Replacement Diploma/Certificate form must be completed in full and notarized. The form can be submitted to a student success specialist on any campus or mailed to the following address:

Cost and Payment

The cost of ordering a replacement of diploma/certificate is $20. Payment options are as follows:

  • Enclose a check, money order or cashier's check with your mailed form payable to Seminole State College. Do not send cash.
  • If paying with a credit card or in person, first submit the form as instructed above (by mail or in person). The $20 fee will be placed on your MySeminoleState account within 48 hours of Seminole State's receipt of the form. Once you see the fee posted on your MySeminoleState Student Center, please proceed to pay with a credit card or in person at any Seminole State Cashiers Office.
  • Payment must be received and all holds and financial obligations to the College must be cleared before the replacement diploma/certificate is mailed.

Requests may take up to 14 calendar days to process. Processing may take longer than usual during peak registration periods.
