The Request for Replacement Diploma/Certificate form should be used only to request an additional or replacement diploma or certificate.
The Request for Replacement Diploma/Certificate form must be completed and notarized, then either mailed to Seminole State/Enrollment Services Department or submitted to a student success specialist on any campus. If you are submitting the form in person or mailing the form, the fee will be placed on your MySeminoleState account within 48 hours of Seminole State's receipt of the document.
The cost of ordering a replacement of diploma is $20. Payment options are as follows:
Enclose a check, money order or cashier's check payable to Seminole State College. Do not send cash.
If paying with a credit card, first submit the form as instructed above. After the payment is viewable in your MySeminoleState Student Center you can pay with a credit card.
Make a payment in person at any Seminole State Cashiers Office, but only after the form is submitted and the fee has been added to your account.
Payment must be received and all Seminole State student record holds removed before the requested document can be mailed via USPS.
Requests may take up to 14 calendar days to process. During peak times (e.g. post-commencement or peak registration periods), processing may take longer than usual.
