Seminole State Nursing Co-op Partnership with Hospitals

Cooperative Education Internship in Nursing (NUR 2941)

Senior nursing students currently enrolled in NUR 2943C Nursing Process: Practicum at Seminole State may concurrently enroll in a one hour nursing internship cooperative experience with an area hospital. During this cooperative experience, students will gain additional clinical practice time at the facility that they are completing their Practicum experience, while receiving pay from the identified hospital.

All the forms you need to meet the requirements of this course are available online at:

These forms include:

• Planning Packet
• Time Log
• Daily Journal
• Final Report

The files for these forms are available to download and print. They have been created in one of two formats: Microsoft Word Documents or Adobe Acrobat files. Acrobat files require the Adobe Reader Plug-in.

Note: Some of these files are large and may take a few moments to download.

Directions for the Planning Packet (Due Date ________________):

1. After printing this Planning Packet, carefully read and review its content.
2. Complete the forms and obtain the indicated signatures.
3. Be sure that your Seminole State faculty advisor completes the Evaluation Plan section of the Planning Packet by indicating the % of Grade.
4. Make three additional copies of the completed packet.
5. Distribute the copies of the completed packet as follows:
   
   • Hospital Representative
   • Seminole State College Faculty Advisor
   • Seminole State College Cooperative Education Office (C-103)
   • Retain a copy for your records

Note: The Planning Packet must be submitted in hard copy, since signatures are required.

Directions for the Time Log (Due Date ___________________):

1. Complete the Time Log on a weekly basis.
2. Have preceptor verify your work hours by signing and dating the Time Log at the end of your cooperative education experience.
3. Make three additional copies of the completed Time Log.
4. Distribute the copies of the completed Time Log as follows:
   
   • Hospital Representative
   • Seminole State College Faculty Advisor
   • Seminole State College Cooperative Education Office (C-103)
   • Retain a copy for your records

Note: The Time Log must be submitted in hard copy since a signature is required.

Directions for the Daily Log (Due Date ___________________)

1. Complete the Daily Log every day after working at your cooperative education position.
2. Make three additional copies of the completed Daily Log.
3. Distribute the copies of the completed Daily Log as follows:
   • Hospital Representative
   • Seminole State College Faculty Advisor
   • Seminole State College Cooperative Education Office (C-103)
   • Retain a copy for your records

Note: The Daily Log may be submitted electronically if this method is preferred by recipients.

Directions for the Final Report (Due Date ___________________):

1. Complete the Final Report after you complete your entire co-op work period.
2. Make three additional copies of the completed Final Report.
3. Distribute the copies of the completed Final Report as follows:
   • Hospital Representative
   • Seminole State College Faculty Advisor
   • Seminole State College Cooperative Education Office (C-103)
   • Retain a copy for your records

Note: The Final Report may be submitted electronically if this method is preferred by recipients.

Directions for the Thank You Note (Due Date ___________________):

1. Write Thank You Note to Unit Manager.
2. Make three copies of the Thank You Note.
3. Distribute the copies of the Thank You Note as follows:
   • Original to Unit Manager
   • Seminole State College Faculty Advisor
   • Seminole State College Cooperative Education Office (C-103)
   • Hospital Representative in Charge of Workforce Planning
