Retired Employee Basic Life Rate	$2.20 per $1,000 of benefit; maximum benefit of $5,000 is $11.00 per month.
Retired Employee Basic AD&D Rate	$.04 per $1,000 of benefit; maximum benefit of $5,000 is $.20 per month. The total cost of basic life with AD&D is $11.20 per month.
Retired Employee Basic Supplemental Life Rate (with age reduction)	$2.20 per $1,000 of benefit (coverage of $5,000, $10,000, $15,000, $20,000 or $25,000).
