Spplemental Health Insurance (Plan C) For Employees Covered Under Other Health Plans

Plan C is a supplemental plan for employees who already have adequate health insurance coverage. There are no employee premiums. Dependent coverage is only available for dental coverage. The plan includes hospital indemnity, dental, and vision coverage only.

Dental Coverage This is the same coverage as detailed in the Dental Insurance. Employees covered in Plan C may cover their dependents for the dental plan.

Maximum Payment for Each Lens: 
  • Single Vision Rx

  • Bifocal Rx

  • Trifocal Rx 
 $40
Maximum Payment for Frames $20
Maximum Payment for Contacts (elected): $75
Hospital Indemnity Coverage Pays the employee $100 per day for each day the employee is confined to the hospital as an inpatient for up to 90 days of continuous confinement.
