AFLAC Voluntary Hospital Indemnity Plan (Level Two)
- Pays money directly to you for hospital confinement, surgery, rehabilitation, large cash benefit for heart attack, stroke, coma or paralysis, also pays ambulance benefits.
- Pays $100/day for the 1st 7 days of hospital confinement;
- $200/day for days 8-30;
- $400/day from day 31-day to day 180.
- $100/day for rehabilitation unit.
- $2000 for 1st heart attack/stroke/coma, or paralysis, $1000 for reoccurrences.
- Wellness benefit of $50/year;
- Ambulance Benefit up to $1000
- Surgical benefit up to $1000
