  • $600/day -- days 1 through 7
  • $1000/day -- days 8 through 15
  • $250/day -- days 15 through 30
  • $250/day -- up to 15 days-step-down unit
  • $25,000 transplant benefit
  • Up to $2000 Ambulance benefit
