This page contains links to the forms and documentation required by Seminole State College's Athletics Department and its regulatory committees for competitive eligibility. Current and future Seminole State student-athletes are invited to use this page as a resource.

The files below have been created in PDF format and require the Adobe Reader Plug-in. This plug-in is pre-installed on most new browsers. If your browser does not already have the plug-in, you can download it for free on the Adobe website.

Health Information Packet

All new and returning athletes must submit a completed Health Information Packet before being cleared for participation in their recruited sport. Below are links to the complete packet, as well as the individual form sets.

The files have been updated for the 2010-11 academic year. Addendums will be posted as a separate link following the list of original documents.
